The Partnerships for Health project involves partners from selected primary care and family health teams and community hospitals within the south west along with the South West CCAC, the Thames Valley Family Practice Research Unit and the South West LHIN. The partners will work co-operatively and beyond traditional boundaries to create a more effective on-the-ground treatment climate and team approach to diabetes management.
