About South West CCAC


Community Care Access Centres are local organizations established in 1996 by the Ministry of Health and Long-term Care to provide access to government-funded home and community services, and long-term care homes. The South West CCAC is one of 14 in Ontario working together with community partners to enhance access and coordination for people across the province.

The South West CCAC serves nearly one million people living in Grey Bruce, Perth, Huron, Middlesex, the City of London, Elgin, Oxford and part of Norfolk County. We help people to:

  • Live independently at home

  • Apply for admission to a long-term care home

We also provide information about local Community Support Agencies and link people with these services. Anyone can make a referral to a CCAC – an individual who needs services, or a family member, caregiver, friend, physician or other health care professional.
